Patient records:

Patients’ records may be inspected for routine audit, training and quality reasons.

Please state in writing if you do not consent to this – this would not affect your care.

The NHS Spine:

The Spine is a set of national services used by the NHS Care Record Service. These include:

bulletThe Personal Demographics Service (PDS), which stores demographic information about each patient and their NHS Number. Patients cannot opt-out from this component of the spine.
bulletThe Summary Care Record (SCR). The Summary Care Record is a summary of patient's clinical information, such as allergies and adverse reactions to medicine. The purpose of the database is to make patient data readily available anywhere that the patient seeks treatment, for example if they are staying away from their home town. Implementation began in 2010, but was suspended on April 16 by the Department of Health.
bulletThe Secondary Uses Service (SUS), which uses data from patient records to provide anonymised business reports and statistics for research, planning and public health delivery.

Patients may opt out of the summary care record by making a request in writing to their GP.
