treatment of prostate problems including prostate cancer (the most common
form of cancer amongst men) with a question and answer session and
patient/wife testimony.

Quality Practice Award submitted 30/06/08.Some minor
details need refining. Assessors will be visiting end-October 2008.

New opening time proving very popular with patients,
especially evening sessions until 8:30pm. The practice are starting to get
used to the "change in rhythm" of the new working arrangements. A few
minor changes will be taking place from 1/11/08 as the working programme
is too tight. We hope to have a few PN sessions in the evening too from
the New Year.

The two new receptionists, Helen & Debbie, are
proving an asset and settling in well.

Two other receptionists, Carol & Sarah, are currently
training to take over the Phlebotomy clinic. From 1/10/08, we will offer

Green issues: Walking to surgery wording for TV
screen:
"Walk your way to health: Your Practice is keen to
encourage as green & healthy approach to attending the medical centre as
possible. Have you thought of walking? Besides improving your health and
fitness, this would also free up space in a congested car-park for those
who do need to attend by car. Thank you."

Out of Hours:
Practice will need to open until 18:30hrs from
1/4/2008. The additional half an hour will be for reception functions and
genuine emergency consultations only. The PPG broadly agreed this was a
pointless exercise and practice expense.
The Government also want practices to stay open
across the lunchtime period. There is an exemption for smaller practices,
which the GVMP intends to adopt, as this provides for lunch-time breaks,
administration without interruptions and staff meetings. The PPG supported
this approach.
We were considering opening late one day per week
until 8pm, should the PCT fund this (cost ~£12K p.a.). However, te
Government are seeking to impose this without additional funding, a move
the practice will be resisting. The PPG understood the issues involved.
However, there are 3 months of negotiations between
now and 1/4/08, so watch this space.

2/ Wednesday 23/11/2005 to discuss "What the
Practice does & how it gets paid". A short powerpoint is available (Click

3/ Wednesday 25/01/2006 to discuss "Medical Ethics".
Members of the PPG discussed a range of scenarios that GPs and patients
might encounter during a typical working day! The scenarios are
available on-line (Click
here).
As each scenario unfolded, members realised that their
decisions/choices were perhaps more influenced by their live experiences
(subjective approach) whereas the GP must endeavour to remain unbiased and
work according to a code of ethics (objective approach).
The discussion was reported to be lively, thought provoking and
enjoyable.

4/ Wednesday 15/03/2005 to discuss the Improving Practice
Questionnaire performed in December by 300 patients. Members
of the PPG discussed the results. The final report has been sent to the
North Eastern PCT for their information.

technology, the potential ethical problems and possible benefits.

9/ The PPG group met on Wednesday 24/01/07 to discuss the handing over to
the Patient Chair, the future vision of the group and to discuss two
patient issues: -
a) Whether to add dissent codes to all patient records regarding the
forthcoming upload of notes from the practice computer system to the NHS
Care Record Spine (NCRS).
At present, patients are able to inform the Practice Manager in writing
requesting that their records are not uploaded to the NCRS. The patient's
computer notes are then tagged with the relevant dissent code, effectively
blocking the upload.
This would effectively convert the Government's "opt out" to an "opt
in" approach. The practice's patients would be contacted advising them of
the situation and patients would then inform the Practice Manager should
they wish to opt-in. The dissent code would then be removed.
The pros and cons of this proposal were discussed, and the PPG voted to
maintain the status quo: I.e. The system is still one of "opt out".
So if patients do not wish for parts their computer records to be
uploaded to the NCRS, they should contact the Practice Manager in writing
to request otherwise.
b) "Best for you, best for your baby?". This is the consultation
on proposals for community midwifery services being conducted by
Chesterfield Royal Hospital.

Essentially, there are 4 proposals for the service reconfiguration (but
do read the full consultation document (takes about 15 minutes):
Location:
To improve the care women receive before they have their baby, CRH are
looking to set up four new maternity bases.
These will provide dedicated antenatal care services (postnatal care
will continue to be provided as it is now). To make sure women across
North Derbyshire have fair access, CRH propose that the four bases should
cover the areas of: -

 | North East Derbyshire |
 | Derbyshire Dales and High Peak* |
 | Bolsover |
Births:
The focus of our midwifery and maternity service is around safe care for
both 'mum' and baby.
To ensure CRH can provide women with the safest clinical service
throughout labour and delivery we propose to offer a choice of: -
 | Home births, or |
 | Births at Chesterfield Royal Hospital |
To enable CRH to offer the choice of home birth (for women where this
is considered an appropriate clinical option) midwives will provide
mums-to-be with more education and support.
Staffing:
To provide community midwifery services in this way CRH will need 41 posts
- a net decrease of around nine from the current establishment.
The biggest change in staffing is a proposal to introduce a new role -
trained maternity care support workers. These members of staff will
relieve midwives of routine tasks and administrative duties – freeing up
time for specialist midwifery care.
Caseloads:
By adopting these new ways of working CRH
will be able to make sure midwives are all working to a more balanced
caseload – and one that compares to similar community midwifery services
elsewhere (between 80 and 100 cases for each midwife).
The proposals were discussed and it was agreed that the PPG would
consult widely with a cross section of society for a range of views,
before discussing more fully at the next PPG meeting.

The PPG discussed the issue of telephone confidentiality and decided
that it was both morally reasonable and practical to leave a message for a
person to call back, so long as no medical details were disclosed (breach
of confidentiality).
There may be a few occasions where some distress might be caused, but
this was considered minimal and preferable to the levels of distress
likely if details (including number) withheld, or if people were not in
receipt of information quickly.
